Near North Health Service Corporation has begun distributing the SARS-CoV-2 (COVID-19) vaccine to healthcare workers. AT THIS TIME THE CITY HAS ONLY PROVIDED NEAR NORTH WITH VACCINE TO ADMINISTER TO TIER 1A (health care workers). Only health care workers may sign up to receive the vaccine at this time. Near North plans to vaccinate the public as soon as the city of Chicago distributes vaccine for the next tier.

To receive your vaccine at Near North, please fill out the registration form below, and visit the Eventbrite link that corresponds to the location you will be visiting to select your appointment date/time. You MUST complete a registration form and schedule through Eventbrite in order to receive the vaccine.

At this time, Near North will be administering the Moderna vaccine. Two doses of the vaccine are required. You must be available to receive the second dose of the Moderna vaccine 4 weeks from receiving the first vaccine, at the same location. You may be required to wait up to 30 minutes after receiving the vaccine to be observed for possible reactions. This determination will be made after answering a series of screening questions. If you have a history of anaphylaxis for any reason, you will be required to wait for 30 minutes after receiving the vaccine for observation.

 

You should NOT receive the vaccine if:

1. You have had any other vaccine in the 14 days prior to your vaccine appointment

2. You have had an allergic reaction to any component of the Moderna vaccine including polysorbate and polyethyleneglycol
